
NJ Paid Family Leave Application |
Employers must make a paper copy of the Application for Family Leave Insurance Benefits available to employees who are planning to take paid family leave.
The employer must complete Part D of the application.
It is the employee's responsibility to file this claim form promptly after they have stopped working and begun taking family leave. The law also requires that employees file their claims within 30 days after the beginning of their family leave period.
